MindFirst (“the practice,” operated by Bernardi Salzman Physicians PLLC) respects your privacy. This policy describes what information this website collects, how it is used, and the choices available to you. It applies to this website only; information you share as a patient of the practice is governed separately by our Notice of Privacy Practices, provided at intake.
Information we collect
This website is deliberately minimal in what it collects:
- Information you send us. If you email the practice or request an appointment, we receive the information you choose to provide, such as your name, contact details, and the content of your message.
- Appointment booking. Appointments are scheduled through Square, Inc. Information you enter in the booking flow is collected and processed by Square under its own privacy policy. We receive the booking details needed to schedule your appointment.
- Patient access codes. Access codes for the practice’s assessment services are issued by email after booking and payment through Square. If you enter an access code in the Patient Access section, the code is used to direct you to the appropriate service. Assessment data handled within our clinical tools is addressed at intake, not by this website.
- Technical information. Our hosting provider may automatically log standard technical data (such as IP address, browser type, and pages requested) for security and operations.
- During verbal tasks in MindFirst Neuropsychology, your browser’s speech-recognition service (provided by Google in Chrome/Edge) transcribes your speech; live audio transits Google’s servers under Google’s privacy terms. Test data files and audio recordings are saved to your own computer and transmitted securely (encrypted in transit) to the practice for clinical scoring; they are not shared with anyone else.
This website does not use advertising trackers, analytics pixels, or social-media tracking technologies, and does not sell personal information. Because we set no non-essential cookies, no cookie consent banner is required.
How we use information
- To respond to your inquiries and schedule appointments;
- To provide and administer clinical services you request;
- To maintain the security and operation of the website;
- To comply with legal obligations.
We do not use website information for advertising, and we do not share it with third parties except service providers necessary to operate the site and scheduling (such as our host and Square), or as required by law.
Health information
Please do not send detailed clinical information through this website or by email. Once you become a patient, protected health information is handled under HIPAA and our Notice of Privacy Practices.
Your rights
Depending on where you live, you may have rights to access, correct, or delete personal information we hold about you, and to ask what categories of information we collect (California residents: California Civil Code §1798.100 et seq.). We honor such requests where the law provides them; write to us at the address below. We do not sell or share personal information as those terms are defined by the California Consumer Privacy Act, and we treat browser-based opt-out signals accordingly.
Data retention and security
We retain correspondence and booking information only as long as reasonably necessary for the purposes above and as required by law, and we use reasonable safeguards appropriate to a small medical practice.
Children
This website is not directed to children under 13, and we do not knowingly collect their information through it.
